How does Interventional Radiology help Diabetic Foot patient & Peripheral Vascular disease patients? Earlier patients with Diabetic Foot had 2 options: 1) Medical Option 2) Surgical Option. Now there’s newly introduced 3rd option – Interventional Radiology. Now what we do in Interventional Radiology is patient with Diabetic foot or peripheral vascular disease patients, due to plaque built-up, Therosclerosis the blood vessels start to shrink and the circumference of blood vessels starts to shrink and estentially over a period of time the entire vessel gets blocked. So circulation starts reducing to foot, hands or everywhere else. Diabetic foot examination:- If abnormalities are identified in monofilament/ vibration sensation then carry out further testing of other sensory modalities (Proprioception/ Reflexes) otherwise move onto assess GAIT. To complete the examination : • Full neurovascular exam • Capillary Blood Glucose • Advice on foot care. In Interventional Radiology through use of small needle and set of wires and catheters we enter the patients body system, cross those blocks and start putting a balloon and opening the blood supply. And in some cases we even deploy a stent, essentially you can think it as cardiology applied to the blood vessels of your lower limbs and upper limbs. The advantage of Interventional Radiology especially for diabetic foot is that :- 1. We avoid a very expensive and very invasive and high mortality related open vascular surgery. 2. Previously we used to amputate literally all patients with diabetic foot but now with the help of Interventional Radiology we avoid that. Once, you start deploying the stent, once you start opening up the blood supply in the blood vessels patient will instantly feel that relief in the pain that he used to feel. And the wheel chair bound patient literally gets up and walks back home. So basically we avoid amputation in diabetic foot patient. So this is the main advantange for Diabetic patients.
